This paper discusses the use of marsupialization as a conservative approach to treat dentigerous cysts, particularly those associated with impacted teeth, to reduce the risk of pathological fractures and preserve surrounding vital structures. The case report details a 40-year-old male who was diagnosed with a dentigerous cyst in the left mandible. The patient underwent marsupialization, followed by tooth extractions and odontectomy. After 1.5 years, an emerging tooth (33) caused suboptimal function of the obturator, leading to its extraction and the fabrication of a new obturator. The case highlights the effectiveness of marsupialization in managing large cysts while preserving bone, though it emphasizes the importance of long-term follow-up due to the possibility of recurrence. The study concludes that marsupialization combined with odontectomy and tooth extraction is an effective treatment, but continued monitoring is essential for optimal outcomes.
